Please note that standard message and data rates may apply, depending on your carrier's pricing plan. These fees may vary if the message is sent domestically or internationally. JG Quality Health Assurance LLC is not responsible for any charges incurred by your mobile carrier.

You may opt in to receive SMS messages from JG Quality Health Assurance LLC in the following ways:

  • Verbally, during a consultation or phone conversation with Jesse
  • By submitting the online contact form on our website and checking the SMS consent box
  • By filling out a paper consent form at our office
